What 4 labels per US Letter sheet means
A count of 4 per sheet does not fix the label size: the 48 sheets here use 21 sizes and 4 grids (2 × 2, 4 × 1, 1 × 4 and 2 blocks). Labels cover between 4 % and 100 % of the US Letter page; the rest is margin and gaps.
Always choose the template by the code printed on the box: two 4-up sheets with different sizes or margins need different templates, even though the count matches. The most common size at this count is 0.219″ × 4 11/16″ (22 sheets).

Which 4-up sheet to pick
Avery 5168 is the default here because Avery codes are the usual reference for compatible sheets, and it shares its exact layout with 3 other sheets on this list, so its template prints on those too. Its labels are 3 1/2″ (88.9 mm) by 5″ (127 mm).
If you need bigger labels at the same count, the largest are 4 1/4″ × 5 1/2″ on OnlineLabels OL450; the smallest are 0.219″ × 4 11/16″ on Avery 5931-Spine. LabelGrid recommends 4-up sheets for shipping labels, wine bottle labels and moving box labels.
